Reducing Bundled Payment Risk
Our unique cloud-based software and unmatched expertise help mitigate the risk of bundled payments for hospital systems, physician groups, insurance companies and Accountable Care Organizations (ACOs).
With Pagati’s intuitive 360° view of hospital readmissions and post-acute spending, transition care teams can confidently design better patient outcomes and more cost-effective care journeys.
As Medicare and insurance companies begin to make payment for medical services dependent on patient outcomes, hospitals and providers are having to take on the risk of a patient’s whole healing journey. That risk is driving a need for more efficient clinical approaches, fewer service redundancies, and partnerships with the best possible post-acute care providers.
The ultimate goal for all stakeholders is better outcomes and reduced readmissions.
That’s where Pagati comes in.
We’ve been a leader in post-acute healthcare for a long time.
Decades of firsthand experience, applied with our unique technology, were the key to achieving unprecedented success in Medicare’s original bundled payment pilot program. Our tools provide an intuitive 360° view of where risk-bearing entities are exposed and where there are opportunities for improvement. Pagati provides transition care teams the necessary data to design outcome-appropriate and cost-effective patient pathways.
Soon after our own experience and success, we understood just how badly other organizations needed the same guidance, strategies and metrics to be successful in this new risk-based payment world.
In 2018, we began helping other hospitals, physician groups and Accountable Care Organizations (ACOs) to develop strong post-acute care strategies to reduce their financial exposure using our cloud-based data analytics solution and hands-on guidance. That mission continues today, as we help risk-bearing entities across the country to navigate the changing landscape of bundled healthcare.
Expertise in data analytics creates better, high-quality outcomes through more cost-efficient patient journeys.
Success rooted in personal service, unparalleled experience and a commitment to understanding each customer’s unique mission.
Partnerships made to go the distance with transparent work and collaboration for the issues impacting you both today and tomorrow.
Our commitment to every client, no matter where they’re positioned on the spectrum of care:
- Data that accurately reflects your financial risk
- Opportunities to improve post-acute performance
- Intelligent tools that can improve both patient outcomes and your bottom line
